Dear Virginia Healthy Pathways Coalition Member: 
I am requesting your assistance in identifying individuals who might be
interested in participating in focus groups as a part of the heart
disease and stroke prevention 
state plan revision process. Please read the attached letter.  You can
assist in one of two ways: 

*	Provide contact information for potential participants in the
matrix provided. 
*	Distribute the letter and matrix to others who could assist us
in identifying potential focus group participants.
