Guidelines for Documenting Skill Proficiency
--------------------------------------------

1. ANY skill listed which has been successfully performed in the field may be 
signed off providing the following:
     a. The skill must have been performed within the last 12 month period.
     b. The skill must have been observed by another provider who is
        certified to perform the skill being evaluated.
     c. The provider observing the skill must initial the appropriate blank 
        on the ALS Skills Form.  The incident number is also needed.

2. You will only need to perform those skills which have not been completed as 
   explained above.

The BOTTOM LINE: The more skills you have signed off, the better!
